Health Aknowledgement

I am not aware of any injury, illness, or other health related issues that would restrict or limit my child's ability to play competitive sports. I agree to assume all risk as a result of my child's involvement in basketball practices, games and/or travel to and from said activities. I agree to hold United Elite Basketball Club or anyone acting on its behalf either as a coach, a coaching assistant or administrator harmless in the event of an injury to my child while participating under the supervision of the above.

Food Allergy Acknowledgement

I/we agree to work cooperatively with United Elite Basketball Club to address my child’s food allergies, if applicable, and to find reasonable measures to implement a safe environment. /we further understand and acknowledge, however, that despite such efforts, neither United Elite Basketball Club can absolutely guarantee my/our child will not come into contact with a food allergen and, that in event such contact does occur, United Elite Basketball Club will not be responsible or liable for same.

Likeness Release Acknowledgement

I authorize United Elite Basketball Club to photograph or film my child and consent to the use of my child’s likeness and image in any and all publications, educational materials, research, marketing, advertising, news media, and Web materials. I understand and agree that such materials, including all negatives, positives, digital images, and prints shall become and remain the sole property of United Elite Basketball Club and I shall have no right or title to such items. I agree that the United Elite Basketball Club does not owe me any compensation for the acts that I have consented to in this agreement. I further understand and agree that these materials may be kept on file and used by United Elite Basketball Club for potential future purposes and further agree to release the United Elite Basketball Club from any and all liability arising from or in connection with the taking, use, publication, or dissemination of such materials
